  2. Your telephone number.
  3. Your mailing address.
  4. When you expect to send us your dissertation.
  5. How you would like your name to appear on your certificate.

Alternatively, your dissertation can be sent at he same time as payment  as an email attachment to:  admin@arccstic.plus.com
Remember to include your Name and address  within the dissertation.
